Bloodwork: what to run and when

Which panels are worth running, how to time a draw, and how to read a number against its range — tracking, not diagnosing.

Why bloodwork is the scoreboard

You cannot feel your hematocrit, your lipids, or your liver enzymes. Bloodwork is the only objective readout of how your body is responding, and a trend across several draws says far more than any single number. The goal here is logistics: knowing what to measure, when to measure it, and how to keep a clean record — not interpreting results, which is a conversation for you and a clinician.

The panels worth running

A broad baseline usually pulls from a handful of panels. Add markers specific to what you are running:

  • Hormones — total & free testosterone, estradiol (E2), SHBG, LH/FSH, prolactin.
  • Complete blood count — hematocrit and hemoglobin matter most here.
  • Lipids — total cholesterol, LDL, HDL, triglycerides.
  • Metabolic — liver enzymes (ALT/AST), kidney markers (creatinine/eGFR), fasting glucose or A1c.

Baseline first, then follow-ups

A draw before you start anything is the most valuable one you will ever take — it is the reference every later result is compared against. After that, follow-up draws are usually spaced to catch changes as they develop rather than after the fact. Logging the date of each draw is what turns isolated numbers into a trend line.

Timing the draw

When you draw matters. For injectables, a trough draw (right before your next dose) gives a consistent, comparable low point; a peak draw shows the high end. Pick one convention and stick to it so your numbers are apples-to-apples over time. Many panels are also drawn fasted in the morning — note the conditions of each draw so a swing is real signal, not just a different time of day.

Reading a value against its range

Every result comes with a reference range printed on the report, and that range is the one that matters — it reflects your lab’s specific assay. The tracker flags values outside a common range as a visual cue only. A flag is a prompt to pay attention and discuss, not a diagnosis, and a single out-of-range value is far less meaningful than a direction of travel across draws.

Building the habit

The people who get the most from bloodwork are simply the ones who do it on a schedule and write everything down. Enter each draw the day you get it, keep the units consistent, and let the trend accumulate. Over a few cycles you will have something most people never build: a personal baseline you can actually reason from.
